Advance Care Planning
Advance Care Planning has been defined as a process of formal decision making that aims to help patients establish decisions about future care that take effect when they lose capacity.
In simple terms this is a conversation between people, their families and carers and those looking after them to discuss and record their future wishes and preferences for care- both for what they do want as well as those things that they don’t.
It is a means of improving care for people nearing the end of life and of enabling better planning and provision of care, to help them live well and die well in the place and manner of their choosing. It’s about helping them to live well right to the end of their life by providing the care that we know they want, even if they can no longer speak for themselves.
